The ILM was founded in 1985 as „Stiftung für Lasertechnologien in der Medizin“ by the companies Aesculap and Carl Zeiss. Initially being focused on laser medicine, the scope of the institute was extended by optical metrology in 1995. The Foundation is open to subsequent founders, who want to support the foundation's mission. Currently, in addition to the initial founders, the Richard Wolf GmbH and the university hospital are part in the circle of founders.
The Foundation is headed by a three-man Management Board, which basically directs the Institute. The Chairman of the Management Board is simultaneously full-time director of the Institute and responsible for the operational management. The Management Board reports twice a year to the Board of Trustees, which is the supervisory organ. Its main tasks comprise the appointment and exoneration of the Management Board, the assessment of the business plan and decisions on questions of fundamental nature. Members of he Board of Trustees include representatives of the founders, ministries of the country, the University of Ulm and other universities as well as other personalities from the economy.
